What Are Oral Restrictions?

Oral restrictions, such as tongue ties, occur when the tongue or other oral tissues are too tight, limiting movement. This can lead to a range of problems for both baby and mom, often starting with breastfeeding difficulties but extending far beyond the nursing years.

Here are just some of the long-term effects of untreated oral restrictions:

  • Obstructive sleep apnea
  • Speech delays and disorders
  • Misaligned teeth
  • High, narrow palate (gothic palate)
  • Frequent ear infections (otitis media)
  • Recurrent colds and ear issues
  • Poor oxygen supply, leading to concentration problems
  • Breathing and sleep disturbances
  • Weak or tense facial muscles
  • Poor posture and chronic pain (neck, back, or headaches)

Common Symptoms in Babies

As a mom, you’re in tune with your baby’s behavior. If you notice any of these signs, it might be worth exploring the possibility of an oral restriction:

  • Frequent or marathon breastfeeding sessions (your baby may be feeding often but not getting enough milk)
  • Frustration or fatigue at the breast, where baby seems to struggle or falls asleep before finishing
  • Snoring or noisy breathing during sleep
  • Spitting up, gagging, or reflux that doesn’t seem to improve
  • Unsettled sleep, especially only sleeping in upright positions like in a car seat
  • Gas and tummy troubles causing discomfort
  • Tension in the body, such as arching their back (C-shaped posture) or holding their head at an angle
  • Mouth breathing while lying on their back

Symptoms in Moms

Oral restrictions don’t just affect your baby—they affect you too. Breastfeeding can become painful and frustrating. Here are some signs you might be experiencing:

  • Sore, cracked, or bleeding nipples
  • Flattened or discolored nipples after feeds
  • Recurring mastitis or blocked ducts
  • Low milk supply or oversupply
  • Painful feeding sessions that make breastfeeding stressful

Why Do Oral Restrictions Happen?

In the womb, a baby’s tongue starts attached to the mouth’s floor and naturally separates during development. But sometimes, this process doesn’t fully occur, leading to a tight tongue tie (or other oral restrictions). Though the exact cause is unknown, we do know that oral restrictions can be hereditary and are more common in boys than girls.

How Common Are Tongue Ties?

It’s difficult to say exactly how many babies are affected, as estimates vary from 2% to 15%, with some studies suggesting the rate could be as high as 40%. This wide range is due to varying definitions of tongue ties in studies, but the effects are real for many families.
